If you’re thinking about IMD, you’re already in the right mindset — don’t wait for certainty. However note that this tool has been built as a screening and escalation aid, not a diagnostic tool.

Invasive Meningococcal Disease (IMD) Screening & Escalation Tool

Use this as a cautious clinical prompt tool for possible IMD. This does not rule IMD in or out. If red flags are present, act early, apply respiratory precautions, and pre-alert the receiving emergency department.

1. Clinical Features Present

A non-blanching rash, confusion, collapse, colour change, or significant breathlessness should push you toward urgent IMD/sepsis concern rather than watchful waiting.

2. Exposure & Transmission Context

IMD spreads through respiratory droplets and saliva. It usually needs close, prolonged exposure and does not tend to spread through brief casual contact or simply being in the same room.

Infection Prevention & Control

If IMD is suspected, apply respiratory precautions early and notify the receiving emergency department.

PPE Requirements

• Apron
• Gloves
• Eye protection (goggles or visor)
• Fluid-resistant surgical mask (FRSM)
• For AGPs: PAPR, long-sleeved gown or coveralls, gloves and eye protection

3. Cleaning Requirements

Allow time after patient conveyance to thoroughly decontaminate care equipment and the vehicle floor.
• Use Clinell Universal wipes for equipment and touch points
• Use a chlorine-based product for floor cleaning

4. Staff Exposure Actions

Staff who believe they have been exposed during the infectious period without appropriate PPE should inform their line manager and contact Occupational Health and Wellbeing for advice and follow up.
Infectious period stated here: 7 days before symptoms until 24 hours after starting effective treatment.

Public Health Reporting

Any suspected case of IMD should be reported immediately to Public Health Wales on 0300 00 300 32 by a registered healthcare practitioner.
